The meteoric rise of GLP-1 receptor agonists like semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) has transformed conversations about weight management and metabolic health. In Palm Beach County, where demand has outpaced supply at many pharmacies, local physicians are working to help patients separate evidence from hype.

Dr. Linda Vasquez, an endocrinologist with a practice in Boca Raton, has seen demand for these medications triple over the past year. "Every consultation, regardless of the reason for the visit, eventually turns to a question about GLP-1 drugs," she said. "There's enormous interest, but also significant confusion about who benefits, what the risks are, and how long treatment needs to continue."

What the Evidence Shows

Clinical trials have demonstrated that semaglutide can produce average weight loss of 15 to 17 percent of body weight over 68 weeks, while tirzepatide has shown even greater efficacy at higher doses. Beyond weight loss, these drugs have demonstrated cardiovascular benefits, with the SELECT trial showing a 20 percent reduction in major cardiovascular events in overweight adults.

However, the drugs are not without side effects. Nausea, vomiting, and other gastrointestinal symptoms are common, particularly during dose escalation. Rare but serious concerns include pancreatitis and potential thyroid effects.

Access and Cost Challenges

Insurance coverage remains inconsistent. While most plans cover these medications for type 2 diabetes, coverage for weight management alone varies widely. Without insurance, monthly costs can exceed $1,000, creating significant equity concerns.

Several compounding pharmacies in Palm Beach County have begun offering compounded semaglutide at lower prices, though the FDA has raised safety concerns about these products. Physicians advise patients to use only FDA-approved formulations from licensed pharmacies.
